Deadline

Printing form out with local printer
Without knowing anything about access I volunteered to learn and do it for the church web site and would not ask so much if it were not for a deadline date. this is what I want to do.
Have a form that can be accessed via the internet where parents can give their Name last first and so on address phone plus the name of their childand then be able to print it out a hard copy to sign and mail plus e-mail it when finished. that would look like what's below.


I Tom Jones being legal parent and guardian of Stacey Jones who resides at "address" do herby give my permission for her to attend the trip scheduled to leave the church Monday I also understand that their will be two adults Mr. Painter and his daughter Shirley accompanying them. so on and so ..........


Printing it out is where I am having the most trouble
