Selecting results from query on form

Thank you for this forum.
I am working on a project where I have 133 stagnant questions on a survey (that are grouped within 7 categories) that are to be answered with a Y, N, N/A response.

I am having a difficult time associating the questions with the responses. I have tried running a query based on each category and placing the results on a form with a combo box, but how do I link the answer to that question?

Memnoch, thank you for replying to my question. I have since learned that I need to take Access to another level and learn VBA to solve my problem.
