Question multiple item info request form

I hope I can make this clear enough

what im looking for is a contact form system that works something like a shopping cart, where a visitor can choose, from many different categories and pages, properties with a check box, then at the end of their session go to a form page, fill out their info and submit it with all the property numbers/names theyve chosen. the idea is that for each property request, the user doesnt need to fill out their info.

should I use cookies to store the customer info? or tweak a shopping cart script?
any ideas are welcome
- barring the "stick your head in a microwave" one Ive heard so often lately.
