Urgent! help needed in form creation

Hi,
i currently need help in creating something like a feedback form whereby users can key in their email and comments and send it to me. I need to be able to send a copy of their comments to my email address and another copy to be saved as a text file on the server.

I need the text file's filename to be like 1.txt, 2.txt and so on for every submit button that i clicked. How do i go about doing it?
