CHECKBOX fields with form and Database

CHECKBOX fields with form and Database

I need some advice on the best way to set up a site.

I have several fields in a table:

FirstName
LastName
Company
Address
City
State
Zip
Telephone
Email
Website

If I want the user to have an option to display or not to display the above fields on a web page, would I set up the table with additional fields like below and then in the form have a checkbox for on/off?

Checkbox fields...

FirstNameOnOFF
LastNameOnOFF
CompanyOnOFF
AddressOnOFF
CityOnOFF
StateOnOFF
ZipOnOFF
TelephoneOnOFF
EmailOnOFF
WebsiteOnOFF

Not sure if there is a better way to do this. Certainly appreciate your views.
