Question CSS Form Question

Ok,

This is not an ordinary post neither is it an ordinary circumstance. I am creating a website and registration application for this program with the Boy Scouts. It is called Venturing and is coed and same rules and stuff as Scouts. The idea sounded great to me so I offered to help for free with this great non-profit cause...

The site itself is done and the application is integrating as we speak. I do have one question though...

The group has created hardcopy forms for those who do not or cannot access a computer for some reason. The forms are neat and organized. Now to help out their staff and the adults who actually register the youth I want to make my on-line forms look exactly if not close to the ones on paper. Below is a link to an example form in a Word Document. Is there any way to recreate this setup with HTML Forms? I am using CSS and I figured this may be able to work with maybe a background image or such.

If the users don't have to be able to type in the form online, they are just going to print it out...then you can recreate the effect by using a table. If they need to be able to type in the fields, I think you're out of luck on making it look like that with HTML.
